Attention all employees (GCMC, TGMA, Morrison, BHU, Wound Care), who DID NOT receive the regular Flu Vaccine:

Please go to the link below (via any internet connection) and complete the 2009-2010 Flu Declination Questionnaire.  No password is needed, and it should only take a few minutes - 4 simple question.  please do not submit more than one questionnaire per employee.
